U.S. Healthcare System

How would you as a healthcare executive or policymaker increase the efficiency of the U.S. healthcare system? To answer this question, consider that there are reasons for the current high U.S. healthcare expenditures: The USA does not regulate the price for healthcare services – using, in most cases, a free-market approach to determine prices. The demographics of the U.S. population is skewed towards the elderly and age and cost increase together. The U.S. population as a whole is a nation of people with multiple chronic diseases like HTN and DM and obesity. The consequences of several lifestyle choices contribute to the high cost of chronic care. The U.S. healthcare system does not seem to work as well as other healthcare systems in the world. How are efficiency and effectiveness related? Can you have both? Make sure to provide credible evidence to support your ideas.

Prior to joining UCSF, Dr. Woodruff worked for 13 years at the U.S. Environmental Protection Agency in the Office of Policy as a senior scientist and policy advisor. Listen to this NPR interview with Dr. Woodruff about the role of science in decision-making (particularly as it pertains to climate change) but why science is not the only factor a policymaker must consider. The transcript is available on this website, but you can also click on the blue “Play” button on the left to listen to the 4-minute podcast: https://www.npr.org/2017/01/26/511851695/former-epa-scientist-weighs-in-on-fate-of-climate- science-under-trump Read this news article featuring Dr. Woodruff’s various work on environmental chemicals and pay particular attention to the “Policies for the People” section where Dr. Woodruff explains why public policies are so important: https://www.ucsf.edu/news/2017/06/407416/toxic- exposure-chemicals-are-our-water-food-air-and- furniture?utm_source=email&utm_medium=edu&utm_campaign=environmental%20toxics After reviewing these resources, write a 250 word or more response to the following questions:     1. When asked whether she makes her own shampoo at home, Dr. Woodruff says “Oh, my God, no” and explains that this is why we need the EPA and where policy comes in. What does she mean by this? How can EPA ensure that people don’t need to make their own shampoo to stay safe from chemical exposures? 2. Dr. Woodruff stresses that public policy is about taking the science we have and using it with other non-science factors that are important for making a decision. For instance, if we know that BPA is harmful to human health, policy-makers must still consider other factors before deciding whether to ban or limit population exposures to BPA. Come up with two examples of other factors specific to BPA that a policy-maker might want to consider, and why they would be important. 3. Dr. Woodruff provides an example of California outlawing flame retardants and the subsequent impact on the population. What do these results tell you about the potential impact of wide-scale public policy? 4. If you could ask Dr. Woodruff any one question about environmental health policy, what would you ask?

Medical Bills

It may seem to you that healthcare has been a national topic of debate among political leaders for as long as you can remember. Healthcare has been a policy item and a topic of debate not only in recent times but as far back as the administration of the second U.S. president, John Adams. In 1798, Adams signed legislation requiring that 20 cents per month of a sailor’s paycheck be set aside for covering their medical bills. This represented the first major piece of U.S. healthcare legislation, and the topic of healthcare has been woven into presidential agendas and political debate ever since. As a healthcare professional, you may be called upon to provide expertise, guidance, and/or opinions on healthcare matters as they are debated for inclusion into the new policy. You may also be involved in planning new organizational policies and responses to changes in legislation. Sleep Disorders

Sleep disorders are conditions that result in changes in an individual’s pattern of sleep (Mayo Clinic, 2020). Not surprisingly, a sleep disorder can affect an individual’s overall health, safety, and quality of life. Psychiatric nurse practitioners can treat sleep disorders with psychopharmacologic treatments; however, many of these drugs can have negative effects on other aspects of a patient’s health and well-being. Additionally, while psychopharmacologic treatments may be able to address issues with sleep, they can also exert potential challenges with waking patterns. Thus, it is important for the psychiatric nurse practitioner to carefully evaluate the best psychopharmacologic treatments for patients that present with sleep/wake disorders.Reference: Mayo Clinic. Value of Stakeholder Analysis

DiscussionsM6D1: What’s the Value of Stakeholder Analysis? FALL – 15 WEEK 2020 Home Announcements Modules Grades People Conferences Writing and Anti-Plagiarism Participation Tool Excelsior College Library Guide TutorMe Support Resources Manage Discussion This is a graded discussion: 0 points possible due – M6D1: What’s the Value of Stakeholder Analysis? No unread replies. No replies. This discussion will simulate a face to face discussion in a classroom. We will go through the questions as a group beginning with the first question. As we answer each of the questions, we will move to the next one together. Read what others have said and add to the discussion in a substantive manner. The first question in this discussion is opinion and experience-based, so you can begin immediately. For subsequent questions please meaningfully integrate what you learned from the videos and articles as well as additional supportive articles or videos that you choose to include. In your opinion, based on what you have learned throughout your studies:   Often, health professionals rely more heavily on their experiences than on data gathering. Practice wisdom is often the gold standard for providers. Compiling a comprehensive list of potential stakeholders in a situation and their perspectives can seem daunting or unnecessary. Suppose a health professional said: “Why should we waste our time analyzing the various stakeholder groups? I have years of practice to draw on.” How would you respond? Now, read the assigned book chapter on stakeholder analysis  (Links to an external site.)  and then engage in a conversation on the following discussion questions, one at a time: The article discusses the role of data collection in the process of conducting a stakeholder analysis. As you read about data collection, what techniques do you feel are most valuable in gathering this information? Why? What techniques might be less useful or yield information that might be biased? Why? Sometimes, stakeholder groups are clearly at odds with each other. How can we deal with these differences? Is there a way to acknowledge these differences and come to a solution? Consult the Discussion Posting Guide for information about writing your discussion posts. It is recommended that you write your post in a document first. Check your work and correct any spelling or grammatical errors. When you are ready to make your initial post, click on “Reply.” Then copy/paste the text into the message field, and click “Post Reply.”  To respond to a peer, click “Reply” beneath her or his post and continue as with an initial post. This discussion will be graded using the SHS Discussion rubric. Please review this rubric prior to beginning the discussion. You can view the rubric on the Course Rubrics page within the Start Here module. All discussions combined are worth 25% of your final course grade. Participation in this discussion assesses the following module outcomes: Evaluate the needs and perceptions of diverse stakeholder groups in addressing health issues.
