This is the culminating project for the course. You will build and present on a population health project that is relevant to you and your career goals. At minimum, this will consist of a paper and presentation. Your assignments through this course can be used to build your project. This will be discussed heavily throughout the course. Each student is required to build a Community Health Education/Promotion program and evaluation as their course project.  This includes an online presentation in week 13

Describe some of the legal and ethical aspects associated with consent. What is the role of the organization with regards to consent? It is well known that smoking is bad for your health. The health education about smoking has always centered around smoking tobacco. Now that marijuana for recreational use is legal in several states, the incidence of smoking is rapidly rising. In addition, smoking alternative products, such as those in vaping, is also on the rise. Your research project this is to research smoking and the autonomic system. A goal of Healthy People 2020 is to “Achieve health equity, eliminate disparities, and improve the health of all groups.”  Read more on disparities: https://www.healthypeople.gov/2020/about/foundation-health-measures/Disparities   The U.S. Health and Human Services Department focuses on six health outcomes to assess health disparities in the U.S.  Choose one of these health outcomes: infant mortality, cancer screening and management, cardiovascular disease, diabetes, HIV/AIDS, immunizations.  Discuss the determinants associated with a positive or negative outcome.  What are the specific health disparities that exist? Use rates to show the difference between the populations from primary sources. Include at least one peer-reviewed journal article on the health determinants you identify.

Final Project Paper Continue the midterm paper adding these dimensions: managing health care professionals; teamwork; cultural proficiency; and ethics and law.   The final paper, which is a combination of your submission at midterm with new sections added, should be 25-30 double-spaced pages in length (excluding the cover and reference pages), written in APA format utilizing 12 point Times New Roman font.

Current trends in healthcare, the recent recession and sluggish economic environment, and the passage of the Affordable Care Act have forced the U.S. healthcare system to develop better ways to provide effective care for less cost. Healthcare providers and facilities must demonstrate to the government, payers, and the public that they provide care that is high quality, safe, and at a reasonable cost. Becoming a healthcare provider of choice while closely managing the costs of doing business and ensuring appropriate financial reimbursements is the new reality for organizations and providers in the healthcare arena. This assessment challenges you to demonstrate understanding of today’s complex healthcare environment while using the business and financial skills learned to develop a strategic plan to move your organization from the world of fee-for-service (FFS) to value based purchasing (VBP). You will evaluate your organization’s position and strategy for the implementation of VBP requirements. You will need to identify the key stakeholders that would be involved (e.g., board members, senior leadership team, physicians, directors, managers, staff, patients, and families).
