Care coordination promotes the provision of patient-centered care that seeks to meet the needs of patients, consider their preferences, and address their concerns. Nurses work with other healthcare providers in identifying the healthcare issues affecting patients, determining the clinical resources required in managing the problems facing their patients, and using evidence-based approaches in ensuring the decisions made promote the recovery and well-being of the patients. Scope of Practice and Roles of Healthcare Team Members The scope of nursing practice is based on the wide range of roles played by nurses in healthcare, their responsibilities to patients and their colleagues and other activities that they engage in that characterize them as nurses. Identified Problem The case that would be focused on in the project would be based on the experiences drawn from the journal that addressed the relationship between the preceptor, Bianca, (the charge nurse) and her relationship with the rest of the staff that she was responsible for in the clinical setting. The problem identified was based on staffing and admission of patients on the unit. The Operations Administrator (OA) that was working in the clinical setting was focused on admitting more patients to the unit without considering whether the unit was full and that the nurses working there would not be able to handle the workload. Although the patients needed care, COORDINATION OF PATIENT CARE PROJECT 4 Bianca believed that it was essential to maintain proper nurse to patient ratio to promote the quality of services offered by the nurses and to ensure that the nurses did not overwork themselves. Additionally, the challenge of communication and collaboration between the OA and the preceptor also formed part of the problem, as the OA was insistent on admitting more patients to the unit without regarding the well-being of the nurses working there. The key stakeholders involved in the unit included the OA, the preceptor (charge nurse), nurses, and nursing assistants working in the unit. The OA was responsible for assigning patients to different units at the point of admission and should work collaboratively with the preceptor and other nurse leaders in determining if the units could handle the increased patient load. The preceptor (the charge nurse) was responsible for delegating assignments to the different nurses working on the unit to ensure that the ratio of nurses to patients allowed nurses to focus on their patients without overworking themselves. The subordinate nurses working under the preceptor included registered nurses and nursing assistants, whose roles involved caring for patients, practicing patient advocacy, and working closely with the patients’ family members in meeting the needs of the patients. Interprofessional collaboration in clinical settings is based on coordination between members of healthcare teams in ensuring that patients are cared for properly and other roles in clinical settings are performed well. The effectiveness of these teams is based on the commitment of each member in performing their roles (Green & Johnson, 2015). In terms of interprofessional collaboration, the preceptor worked closely with the OA in ensuring that the OA understood that the unit was full and could not admit more patients based on the limited ratio of nurses to patients. The preceptor also worked with the rest of the nurses in the unit in ensuring that there was proper allocation of patients to the nurses and that a system of allocating new COORDINATION OF PATIENT CARE PROJECT 5 patients to the nurses after discharging a patient from the unit was used. The system ensured that the nurses had good working conditions and that a proper ratio of patients to nurses was maintained. While interprofessional collaboration is essential in clinical practice, understanding and maintaining the boundaries that differentiate the roles performed by different professionals is important. Boundaries in clinical practice are upheld through role clarification. For instance, in the case of the preceptor and the OA, the role of the OA was the allocate patients to different units based on their condition while the role of the preceptor was to allocate the patients admitted in the unit to nurses to ensure that each patient was well cared for by a specific nurse. The preceptor’s role ensured that the nurses working in the unit were not overburdened and maintains a proper nurse-to-patient ratio. Some of the ways that boundaries issues are addressed in literature include the development of role clarification systems, task reallocation, role construction during hiring processes, and clarification of team members’ roles in teams (Niezen & Mathijssen, 2014). In the case identified, by ensuring that more patients were not admitted into the unit, the preceptor prevented the OA from crossing the boundaries of their roles. Other stakeholders who should be involved in addressing boundaries issues include the board of governance in healthcare facilities, leaders like nurse managers placed in charge of different healthcare practitioners, and the human resource manager. Implementation Plan The implementation plan would be based on clearly defining the roles of the different healthcare workers involved in the allocation of patients to units and caring for them. Additional data would entail the healthcare facility’s role allocation plan to determine whether there were roles that overlapped each other. As the charge nurse, role clarification and collaborating with the OA would be the most effective approach of addressing the boundary issue. The plan could be implemented by working with the human resource manager in specifically identifying the roles that need to be played by different healthcare practitioners in the unit. The chart could be integrated into the healthcare facility system and used by nurses to examine their daily roles depending on the patients admitted in the units or the workload (Brault, et al., 2014). The goals of the evaluation method would be to ensure that the healthcare staff have performed the roles scheduled for them, examine the extent to which the staff perform their roles based on the scope of their practice, and examine the outcomes of the patient care regarding the changes implemented by checking for rates of missed care or medication errors in the unit and comparing them to previous months or years. The patients and family members will be involved by requesting them to answer a questionnaire on their experiences at the facility.
