Rated pain 8/10. The nurse stated that she gave her Tylenol 650mg last night and she went to sleep. When she woke up she had trouble with her speech and disoriented. The nurse stated it lasted about 5 minutes, then she was back to herself. Nurse stated that patient was on 81mg Aspirin and she has been refusing them for 3 months. Patient denies dizziness, jaw pain, vision changes, chest pain, shortness of breath, N/V, constipation, diarrhea. Current medications Zocor 20mg, Eldertonic 2 tsp daily, Vitamin D 1 PO weekly, hydrochlorothiazide 50mg, omeprazole 20mg, Aspirin 325mg, Tylenol 650mg. V/S 142/86, p-82, r-16, t-97.6, 97%.Stroke Patient # 2 69-year-old male with HTN, Hyperlipidemia, Hypothyroidism presents with for an annual visit and verbalizes concerns of continued low back pain after he fell four months ago. Pain rated 7/10. Pt has a history of CVA with residual right sided weakness. Patient uses a walker to ambulate and feels that when he walks, he is bent over and strains his back and shoulder. Patient took the walker to prosthetics and had it measured and adjusted without improvement. Pt uses ben-gay and Tylenol for his pain but says that the pain relief only lasts about an hour. Denies chest pain, shortness of breath, dizziness, headache, N/V. Medications patient currently taking are Atrovastatin 20mg, Lopressor 25mg, Norvasc 10mg, Colace 100mg, Eliquis 5mg, Multi-vitamin, Tylenol 650mg.
