This case study focuses on some of the issues that may arise from changes to how health information is gathered, specifically changing an electronic clinical documentation system. The issues described in this case could apply to changes arising from health information sources such as a new health care law, updates to forms, new reporting requirements, changing from ICD-9 to ICD-10, and so on.
