Please use this an EXAMPLE of how to frame your statement of appeal incorporating the required explanations: (i) verifiable documentation of circumstances that contributed to the withdrawal/dismissal, (ii) an explanation of how these circumstances have been resolved and (iii) provision of a reasonable plan for meeting the academic requirements for good standing. The statement and your supporting documents are the only documents you have to state your case for returning to your program of study.
