Statement of Purpose: This research paper will discover how to manage virtually all the poorly handled, lost, misplaced health information, The aim is to retain and manage this information using electronic health record(EHR) to handle more securely ou patients/ individuals information. Thesis statement: to resolve the problem of individual, patients confidentiality being violated, abused, information being lost, misplaced, improperly filling by staff- health officers, direct care aide, Pharmacy assistant/ Technician etc, This paper will suggest and stress the importance of in-service training to bring alive the importance of maintenance of individuals, patients records, safeguarding of staff password, training of staff on electronic health records device platforms… The objective of the study: Effective communication of individuals, patients health record is essential. it assists care provider in review previous and present care activities and planning and executing care, so having up to date record is vital, secondly, Health information management professional has a vital role to play in maintenance, creation, disposal,  of the health record, the onus of this work is envisioning a time of good management, retention. proper filing,  ease retrieval of the patients’ health record in a more secured device, efficient disposal of patients information free from violation of patient health record confidentiality.
