C) Self-Assessment. This paper has two parts. The first section will be a follow-up to in-class activities presented in Weeks 1 and 2. For this assignment you will reflect on your cultural identity. Honesty is expected and will not be penalized, however a respectful tone is required. The first section should be 2-3 pages APA Style and is an honest assessment of your cultural identity, how you see yourself at the start of this class. We will do a few in-class exercises during the first two weeks to support this process. The second section is due Week 11 and should be 4-6 pages APA Style. This section is to include a self-assessment using Pamela Hays Addressing Model discussed in class along with reflections of changes or new awareness that has emerged as a result of the course. Compare and contrast two change theories, and determine which theory makes the most sense for implementing your specific EBP project. Why? Has your mentor used either theory, and to what result?
The first change theory is Lewins Change Theory. This theory is very widely used in nursing. This theory has three stages the unfreezing stage, moving stage, and refreezing stage. The theory has driving and resistant forces and for the theory to be successful the driving forces have to overcome the resistant forces. The other change theory is Rogers Change Theory. This theory has 5 stages and they are awareness, interest, evaluation, implementation and adoption.(Oguejiofo,2017) It is successful when nurses who ignored the proposed change earlier adopt it because of what they hear from nurses who adopted it initially. Both of these theorys are widely used in nursing and both require nurses that want the change or who are willing to make the change. My mentor has used the Lewins change theory recently. The hospital already has hourly rounding but she just introduced new paperwork that has to be signed every hour. The unfreezing period she just explained how the new way will be better and she showed the nurses how it will be easier because the techs can also sign the sheet. The moving stage she let the nurses tell her how they feel about the whole situation and letting them express what they think will work. The final stage is refreezing and during this stage she went around for the first week making sure the nurses get this in their daily habit. I believe this theory makes more sense to my EBP because there are a lot of nurses that will be the driving force to make this happen and less people being the resistant force. So, it will be more likely to succeed.

While these techniques may have merit, practices are constantly being updated and contradicted by information from scholarly research studies and professional guidelines. This new information serves as evidence for revising practices to improve outcomes across health care.
Based on this evidence, you can formulate a question. In this Discussion, you consider the use of evidence-based practice in your own organization and formulate a question that you will need to answer for your portfolio project. This is called a PICOT question. You will also investigate strategies for overcoming barriers to implementing evidence-based practice (EBP).

Mary is a 47-year-old, single, heterosexual Caucasian female. She lives with her 52-year-old sister and 87-year-old father in the home in which she was raised. She also has a 45-year-old sister who lives 10 minutes away and a 23-year-old daughter living on her own. Mary and her family members do not maintain friendships outside the family. Mary has been unable to work for the past 3 years because she says she has felt too frightened to go too far from her home. As a result, she has been financially reliant on her family members for these last few years. Prior to this lapse in employment, she had been a school bus driver and an administrative assistant at a warehouse distribution center. Mary has no history of drug or alcohol abuse. She is well groomed and physically fit with a diagnosis of hypoactive thyroid, for which she is treated with Synthroid®. Mary was diagnosed with post-traumatic stress disorder (PTSD) and adjustment disorder, not otherwise specified (NOS) by the clinic psychiatrist. Assignment: The Case of Mary
Before meeting with me, Mary saw a social worker in a private practice for 2 years. She entered treatment with that clinician because she said she was traumatized by a romantic relationship with a married African American man she had met at work. Her trauma symptoms began 6 months after she ended the yearlong romantic relationship. Mary said the romance occurred because he had brainwashed her, as there could be no other reason she would have slept with him. Mary believes that bad people are capable of brainwashing good people to perform bad deeds. Mary was raised in a home that espoused racism, and she and her family members believe that African Americans and other people of color are untrustworthy and bad. She said, I take after my father, and he thinks black people are just evil. Mary said she understands her feelings about race are not right.
Mary considered her initial attempt at treatment unsuccessful for two reasons. First, she felt the therapist (a Caucasian woman) judged her and her family harshly for their racial beliefs and this got in the way of the two of them building a trusting working relationship. Second, she did not feel relief from her PTSD symptoms. Mary ended the relationship with that social worker 6 months ago. Mary then approached her primary care physician about her symptoms, and the doctor prescribed an antidepressant. When, after 3 months, Mary asked to have her dosage increased, the doctor suggested that she get a psychiatric evaluation and consider returning to therapy. Marys insurance company suggested our agency for the psychiatric evaluation and approved 10 sessions.
Mary said she felt sad, frightened, and anxious most of the time. She had no appetite, slept most of the day, had no interest in dressing, and rarely left the house. When she did go out, she felt the need to be accompanied by of one of her sisters.
Mary presented as angry during our initial sessions. She requested that one of her sisters attend the sessions with her, to which I agreed. My intent in agreeing to have her sister in the room was to help Mary feel safe and create a strong rapport. During the early sessions, most of what Mary said began as half sentences that she asked her sister to complete. Mary referred to her sisters as her caretakers and minders who know me better than I know myself so you should talk to them. Mary said that if she talked for herself she would get it wrong. The it and the wrong remained elusive in meaning when I asked her what that meant.
Mary agreed, after two sessions, to meet with me alone. We used our first individual session to establish Marys goals for therapy. Among her goals was developing ways she could feel safer about going outside alone. Over the next eight sessions, I used cognitive behavioral therapy interventions to help Mary build coping strategies for recognizing triggers to frightening thoughts and to help her manage her anxiety symptoms. I also used psychoeducational interventions to help Mary develop routines for proper sleep hygiene, healthy eating, and regular exercise.
After several sessions, Mary shared insight into her feelings about dating an African American man. Mary said that being attracted to an African American man frightened her and that there was no future for her relationship with this man because he was married. Mary believed that she had jeopardized her secure position in the family because being with an African American man challenged the familys ideas about race and their view of themselves as separate and unique from non-family members. Once the family discovered Marys relationship with this man, she believed her only way back into their lives was to accept the role of a crazy sister in need of protection and whose judgment about people was faulty. By forming a relationship with an African American man, Mary had shown her judgment to be outside of the norm in the conventions of her family.
In our final two sessions, Mary said that she no longer felt like she was the crazy woman in the family and she felt safe going to the grocery store alone. It was my impression that Mary may have been the identified patient in her family but exploring this idea would require family therapy.

Mental health processes and disorders originate from mechanisms within the brain. There has been the question within the psychiatric field as to whether plastic changes in the brain that can occur with the use of pharmacological interventions for mental health disorders could also occur with the use of psychotherapy. Psychotherapy is extremely beneficial in treating several mental health issues (Laureate Education, 2016).There is compounding evidence that psychotherapy does have a biological basis and can have a positive impact in brain recovery from the stress response. According to Wheeler (2014), psychotherapy mediates the reintegration and connection of neural networks that have become maladaptively linked due to adverse life events facilitating healing of the brain. Psychotherapy has been found to be an effective treatment method for a variety of mental health disorders such as anxiety, major depression, and post-traumatic stress disorder. According to Fournier (2014), activity in regions associated with negative emotion, emotion regulation, fear, and reward are associated with respones to psychotherapy, and psychotherapy appears to alter the functioning of these regions.
While proven to be an effective treatment modality for mental health disorders, there are factors such as culture, religion, and socioeconomic background that can affect the client and their perspective on the use of psychotherapy. Psychotherapy within itself can bring conflicting emotions for the client and the clinician must be aware of how religion, culture, and socioeconomics can alter how the client perceives psychotherapy and its efficacy in their treatment. According to Wheeler (2014), the powerful influence of culture permeates all dimensions of out life in a way that is often unconscious. For example, if a client comes from a culture where emotions are not to be discusssed or if one discusses there past traumas or fears then the client could be resistant to the role of psychotherapy in their treatment plan. The clinician must be aware of the importance of culture in medical or psychological treatments. Religion also plays an instrumental role in how psychotherapy will be perceived by a client. Many clients may come from a religious background where one only speaks of negative emotions with someone from their clergy or a religious figure. According to Kim, Chen & Brachfeld (2018), religion and spirituality are important issues to consider and address in psychotherapy. Communication could become stagnant if the clinician is not aware of the role religion plays in the psychotherapy framework. Socioeconomic background can also be a variable in ones perspective of the value of psychotherapy. Certain traumatic events that a person can suffer throughout their lifetime can be directly correlated to socioeconomic standing. For example, poverty can be associated with depression, anxiety, substance abuse, and mood disorders. Patients from this type of background can experience barriers in both seeking and receiving mental health services. According to Bernal et al. (2017), vulnerable populations such as those low in social status face additional barriers to mental health treatment and experience unique barriers to receiving optimal care.

This is a continuation of the Course Project presented in Week 2. Before you begin, review the Course Project Overview document located in the Week 2 Resources area.
The literature review is a critical piece in the research process because it helps a researcher determine what is currently known about a topic and identify gaps or further questions. Conducting a thorough literature review can be a time-consuming process, but the effort helps establish the foundation for everything that will follow. For this part of your Course Project, you will conduct a brief literature review to find information on the question you developed in Week 2. This will provide you with experience in searching databases and identifying applicable resources.
To prepare:
Review the information in Chapter 5 of the course text, focusing on the steps for conducting a literature review and for compiling your findings.
Using the question you selected in your Week 2 Project (Part 1 of the Course Project), locate 5 or more full-text research articles that are relevant to your PICOT question. Include at least 1 systematic review and 1 integrative review if possible. Use the search tools and techniques mentioned in your readings this week to enhance the comprehensiveness and objectivity of your review. You may gather these articles from any appropriate source, but make sure at least 3 of these articles are available as full-text versions through Walden Librarys databases.
Read through the articles carefully. Eliminate studies that are not appropriate and add others to your list as needed. Although you may include more, you are expected to include a minimum of five articles. Complete a literature review summary table using the Literature Review Summary Table Template located in this weeks Learning Resources.
Prepare to summarize and synthesize the literature using the information on writing a literature review found in Chapter 5 of the course text.
To complete:
Write a 3- to 4-page literature review that includes the following:
A synthesis of what the studies reveal about the current state of knowledge on the question that you developed
Point out inconsistencies and contradictions in the literature and offer possible explanations for inconsistencies.
Preliminary conclusions on whether the evidence provides strong support for a change in practice or whether further research is needed to adequately address your inquiry
Your literature review summary table with all references formatted in correct APA style
Note: Certain aspects of conducting a standard review of literature have not yet been covered in this course. Therefore, while you are invited to critically examine any aspect of the studies (e.g., a studys design, appropriateness of the theoretic framework, data sampling methods), your conclusion should be considered preliminary. Bear in mind that five studies are typically not enough to reflect the full range of knowledge on a particular question and you are not expected to be familiar enough with research methodology to conduct a comprehensive evaluation of all aspects of the studies.
