What are the unmet health care needs you see in your community? What determinants of health are related to these unmet needs and what determinants require action and advocacy efforts? How do your identified issues impact your work and your sphere of influence? Consider some of the following examples that affect communities: various needs of veterans (homelessness, post-traumatic stress), immigration, undocumented workers, medical and/or legalized marijuana, reproductive health care, unemployment, poor environmental conditions, limited access to health care, and lack of heath care. How do these issues impact community health and what is your role in the solution of these issues? How does a particular health determinant impact your practice? (You may choose to address a health determinant that is not listed.) As a nursing professional, what can you do about it? What are several ways that you can advocate for change in this area? Why might you choose to not get professionally involved in this area or in others like it? Note Initial Post: A 3-paragraph (at least 350 words) response. Be sure to use evidence from the readings and include in-text citations. Utilize essay-level writing practice and skills, including the use of transitional material and organizational frames. Case description: You are a public health practitioner working in Australia or Overseas. The Communicable Diseases Department of your State ( or Local) Government are asking for expressions of interest from public health practitioners such as yourself for initiatives aimed at controlling the spread of STIs in a local community. The funding for these initiatives is very broad and can cover clinical services, health education, public health campaigns, health promotion, legislative initiatives or social policy changes. The funding offered is significant and would be enough to establish an impressive health initiative. Many different initiatives are likely to be considered in a broad-based approach to the control of STIs.     Instructions: 1. Title of Project 2. Context, setting and role of stakeholders: explain why you selected this community eg Melbourne ie urban metropolitan ethnically diverse young community or rural African community in Tanzania (literally a few sentences). Discuss any local or national policies that may be related. 5. Justification and evidence for your project ie evidence that it is likely to be effective in achieving its objectives (eg use other similar, ideally published, studies or interventions that have been evaluated as examples) 6. Methodology of initiative (ie how are you going to do it, including evidence or efficacy if not already discussed above). Highlight any relevant difficulties there may be with implementation. Discuss how you could address these. 7. Brief plan of how the project will be evaluated

Discuss the factors relevant to the priority population and community in depth. • Contributing factors: Explain the factors that influence this health problem including: Individual factors: behaviors, skills, attitudes, beliefs, and genetics Environmental factors: natural; social (family, friends, colleagues, community beliefs/attitudes) structural, and built resources and barriers Systemic factors: institutions, policies, biases, cultural values/beliefs Part III: The Impact of the Problem Using data from your social, epidemiological, and educational assessments, describe the impact of the problem. • Consequences and Opportunities: Explain the impact of the problem on people and communities. How does the problem affect: Quality of life (being, belonging, and becoming) Life expectancy Health issue costs – (financial and emotional) • Comorbidities What are the impacts on individuals, families/peers, and the community if nothing is done and what can be expected if there are interventions? Acknowledge both short and long- term impacts. Part IV: Promising Approaches for Improved Results Using data from your social assessment research, describe the potential for change. Capacities: Identify community capacities that can help with change efforts. Explain the capacities, strengths, assets of the priority population that will help foster change, and key resources (organizations/programs/professionals) that are already available, refer to asset mapping. Solutions: If known, introduce best practices/changes that have been shown effective solutions to the problem.
