You would like to implement a strategic plan to improve the health status of your community. Select a vulnerable population in your area affected by a disease or condition. Examples include aging, COVID-19, diabetes, Ebola, heart disease, opioid epidemics, Zikavirus, and so on. Write a three- to five-page paper that details your strategic plan. In your paper, Describe the population, including demographics and risk factors that determine health in this population. Explain the disease or condition prevalent in this population. Identify access and barriers to health care and treatment options for this population, including local, state, and federal policies regulating control and prevention of your selected disease or condition in this population. Identify, analyze, and discuss the components that comprise a valid evidence-based research study. Based on the information compiled above, interview a health care leader about his/her experiences related to using evidence-based research to drive changes in practices in his/her facility. A Medicare Cost Report (MCR) collects descriptive data, both Financial and Statistical in nature to identify if a facility has been over or Underpaid for Medicare patients. The data is also utilized to set Prospective payments.  MCRs are in place for Institutional Providers, not Physicians, and remain the ONLY national data available for all types of providers, Profit, Not for Profit, and Governmental. Typical providers are: Hospitals, SNFs, HHA, Hospice, Renal Dialysis centers, FQHC, and CAH (Critical Access Hospitals) For this week, your assignment is to explore the role the MCR plays in a value-based health care system.  Specifically, address issues included in the report like facility characteristics, utilization data, cost, and charges by cost center.  Furthermore, choose two utilization metrics (data) and address how it has impacted the facility of your choice.
