Studies indicate that health care services provided for medical beneficiaries with five or more chronic conditions make up of about 74% of the total health care cost spent. The majority of these costs are as a result of high rates of avoidable hospitalizations. Healthcare providers, who focus on continuity of care such as transitional nursing aims at enhancing patient’s experience, reduce health care cost, increase population health. Particularly, transitional nursing which is set of limited services that are provided during an acute illness episode across healthcare settings has been recognized as an efficient approach to improve and manage health care services provided to older adults. Evidence-based show that the transitional care model has consistently proven to be efficient in enhancing economic and health outcomes for older patients.
